Firstly, selecting Only their own in the advanced settings shouldn't "grey out" any options in that menu. I have a few questions : 1 - Are you using MOSS? 2 - Do you have anonymous access turned on? 3 - Do you have the lockdown feature turned off? Setting read access for only your own shouldn't block a user from adding a new item. I'm pretty sure this works since the screen shots were taken from a live survey that I created this way. When I set the "Only their own" options in step-1 the option for "Add Items", "View Items" are disabled. However if i set the Read access = All Responses i get the "Add Items", "View Items" in step-3 but it serves no purpose since the anonymous users will be able to see all the responses.
